In this video, Professor Dan Reinstein performs a ReLEx SMILE (small incision lenticule extraction) - a keyhole type of LASIK procedure - filmed in real-time to demonstrate the full 3 and-a-half minute procedure from multiple angles. The superior design and optical precision of the Carl Zeiss Meditec Visumax femtosecond makes it the only femtosecond laser that is accurate enough to perform laser eye surgery through a keyhole incision in the cornea without the need for creating a flap and disruption of the corneal nerves and tearfilm that results from flap creation in the standard LASIK technique. The patient is only in contact with the device is less than 40 seconds with extremely low contract force such that the patient feels effectively nothing, there are no red splodges (subconjunctival haemorages) left behind. From the surgeons' standpoint the laser procedure is one step: contact with the patient cornea and lenticule creation. The accuracy of ReLEx SMILE has been shown to be higher than that of LASIK in that it is far more accurate for higher prescriptions, and ReLEx smile can perform treatment for myopia / short-sightedness from -1.00 to -10 and in some instances up to -14 D. Cataract Surgery With Ophthalmic Microscope | Leica Proveo 8

For more information visit our product page: 🤍 ▬Beginning ▬▬▬▬▬▬▬▬▬▬▬▬ The cataract surgery starts with a 1 mm side port. After that Dr. Fabio Dornelles pressurizes the anterior chamber with dispersive viscoelastic. During this phase of cataract surgery, he prefers to create a self-sealing 2.4 mm CCI, followed by the roundest possible capsulorhexis, typically around 5 mm. This step of the surgery requires an excellent visualization with the best red reflex. Leica Proveo 8 microscope is very capable in this requirement, allowing an incredible red reflex, even after hydro dissection. ▬Rotation ▬▬▬▬▬▬▬▬▬▬▬▬ The next steps are hydrodissection, hydrodelineation, and nucleus rotation. At this step of cataract surgery, the hydro-chop technique is used followed by four quadrants of phacoemulsification. It’s recommended to use a phaco machine with a venturi pump, this speeds up the procedure time. ▬Advantage of Proveo 8 Microscope▬▬▬▬▬▬▬▬▬▬▬▬ During cataract surgery, you don’t need to refocus the microscope so frequently thanks to the FusionOptics feature that gives a greater depth of field. In the most critical steps of cataract surgery, it also helps not to get distracted with the footswitch all the time, focusing only on what really matters - the cataract surgery itself. ▬Epinucleus aspiration ▬▬▬▬▬▬▬▬▬▬▬▬ The next step of cataract surgery is to aspirate epinucleus by the phaco tip. When the cortex is aspirated, Dr. Fabio Dornelles prefers to use the Hurricane technique with a 30-degree coaxial IA. With Leica Proveo 8 microscope, he can observe even the smallest details in high magnification. Posterior capsule polishing is mandatory because we actually see whether the cortical remains attached to the posterior capsule. In this case, Dr. Fabio Dornelles uses a BSS-flush with a hydro dissection cannula to gently clean it. He prefers to mount the IOL under the microscope, as the microscope really speeds up my workflow with the quick focus feature. ▬Last steps ▬▬▬▬▬▬▬▬▬▬▬▬ With the IOL already positioned inside in the capsular bag, Dr. Fabio Dornelles performs the aspiration of the viscoelastic and the incision’s hydration. One man is speaking out about the potential risks of laser eye surgery, after he says the procedure left his vision permanently impaired.

Cataract Surgery in an eye with prior LASIK


With age, everyone who lives long enough will develop a cataract. This means that today's LASIK patients will someday need cataract surgery and the patients who had LASIK two decades ago are nearing cataract age.  The first thing to understand is that surgery must be done differently for these patients. The primary considerations are that the lens power estimation is calculated differently than compared to virgin eyes and that we must be careful not to disturb the corneal LASIK flap.  The error in the lens calculation is usually opposite of the prior LASIK treatment. For example, eyes which have undergone prior myopic LASIK will end up with a hyperopic refractive surprise if we don't adjust the IOL power appropriately. Patients who had prior hyperopic LASIK will end up more myopic than planned after cataract surgery if we are not careful. For more information on IOL calcs, see our website 🤍IOLcalc.com At the time of cataract surgery the LASIK flap edge must be identified and then avoided during creation of the incisions. A simple approach is to let the cornea dry for 30 seconds and then use light touch from a weck-cel sponge to gently press on the cornea. The edge of the flap will present itself and be easily seen. DMEK Eye Surgery


John Barsa, 73, of Parma, Ohio, says he was as “blind as a bat” until he had eye surgery at the UH Eye Institute. John had a procedure in his right eye called DMEK (pronounced DEE-mek), in which a very thin, delicate sheet of corneal cells was transplanted. The sheet of cells is only about one one-hundredth of the thickness of a dime. John was the first patient at UH to have DMEK. His eye surgeon Dr. Pankaj Gupta, who developed the DMEK transplant program for UH, says, “DMEK is probably one of the best things that we can do for our patients. It provides them the best vision possible and it is a surgery that gives me a lot of comfort and joy knowing that at the end of the day, I have a patient that’s going to successfully be able to do a lot of the things that they want to do for the rest of their life.” As for the results, John says, “I forgot how nice colors were because everything was kind of gray. And all colors are vivid to me now and shapes are very, very sharp now. I can make things out with no problem at all; 100 percent better. MSICS: manual small incision cataract surgery


In many countries where dense, brunescent cataracts are common, the SICS (small incision cataract surgery) or MSICS (manual small incision cataract surgery) techniques are very popular. These are upgraded and evolved versions of the traditional manual extra-capsular cataract extraction (ECCE) technique with many important upgrades. The main upgrade is an incision that is carefully shelved so that there is a sufficient tunnel length to provide good closure with few, if any, sutures. The incision is also trapezoidal or funnel shaped with the internal opening larger than the scleral opening. This technique is excellent for truly dense, brunescent cataracts because no ultrasound energy is used to remove the cataract and this can be gentler to the corneal endothelium. Keep in mind that if you are concerned with the refractive outcome of the case, the larger incision used in MSICS will induce far more astigmatism than the typical 2.2 to 2.8 mm phaco incision. In the USA, particularly at our resident training programs, we frequently teach this MSICS technique. This is certainly the case at our program at the Jules Stein Eye Institute and Olive View UCLA Medical Center, both part of the UCLA School of Medicine in Los Angeles. This is also taught at the Bascom Palmer Eye Institute at the University of Miami in Florida and this video is from a resident training at that program.

This requires not only a clear cornea which is the transparent, dome-shaped surface of the eye where light enters, but also a clear lens which is found behind the iris and pupil. Laser Eye Surgery PRK - PreOp Patient Education HD


🤍 and 🤍 Patient Education Company Your doctor has recommended that you undergo Photorefractive Keratectomy - or PRK Laser surgery - to correct a vision problem. But what does that actually mean? The human eye is constructed like a camera ... with a clear lens in the front and light-sensitive tissue at the rear. This tissue makes up the retina which acts like photographic film. In an eye that has perfect vision, light rays passing through the pupil are focused by the lens to fall precisely at the center of the retina. There are many common problems that can affect the eye and prevent light rays from focusing properly on the retina. Three of these problems, myopia - or nearsightedness; hyperopia - or farsightedness; and astigmatism can often be corrected or reduced with the use of PRK laser surgery. Myopia, or nearsightedness, occurs when the shape of the eye is too long or the curve of the cornea is too extreme. In this case, light rays are focused on a point in front of the retina - instead of on the retina itself. Hyperopia, or farsightedness, occurs when the shape of the eye is too short. In this case, light rays are focused on a point behind the retina. Astigmatism occurs when the cornea is unevenly curved, causing light rays to fall off center or not to focus properly at all. In either case, PRK laser surgery can be used to flatten all or part of the cornea ... allowing your doctor to cause the focal point of light entering the eye to fall more closely to the center of the surface of the retina. PRK is a simple and nonintrusive procedure that is designed to reduce or eliminate the need for glasses or contact lenses. PRK laser surgery generally does not have any effect on a patient's overall health and there are no risks in choosing not to have the surgery. Your Procedure: On the day of your operation, you will be asked to put on a surgical gown. You may receive a sedative by mouth and an intravenous line may be put in. You will then be transferred to the operating table. and you'll be given an anesthetic in the form of eye drops and an eyelid holder will be placed around the eye to prevent blinking. When the operative field is numb, the doctor will carefully remove the epithelium, or top layer of cells, exposing the stroma - the non-cellular portion of the cornea. Next, your doctor will use a computer to control pulses of cool laser light. These pulses will delicately remove microscopically thin layers of cells from the stroma. By removing tissue in this manner, your doctor will tailor the new shape of your cornea according to the exact nature of your vision problem. The entire procedure usually takes about 5 minutes and is painless. Following surgery, you'll be given protective contact lenses to wear for a few days while the outer layer of cells grows back.
