Costa brava was going to fly all the way from madrids barajas airports to bostons logan international airport. The plane had 153 passengers and 14 crew members on board . at about 3:54 pm the plane was getting close to to boston and the controllers cleared the plane to get the down to 3000 feet. The controller gave fight 933 vectors to intercept the localizer to runway 33L. At this point they were 9 miles from the outer marker and and minutes later flight 933 was handed over to the tower controllers. As soon as flight 933 was in contact with the tower they got some bad news, the runway visual range system was out of range but the visibility was three quarters of a mile with low winds. So not too bad in the grand scheme of things this plane should be able to land pretty soon. Soon the plane plane was lined up with the runway and the autopilot was engaged with the autopilot , the autopilot was managing speed and the heading of the plane but the plane was prepped for the landing, As the plane got closer the controllers cleared the plane to land, the braking action was fair to poor. That could have been better but since the runway was more than 10000 feet long so stopping should be no problem. As the plane broke through 300 feet the captain disconnected the autopilot, then the approach light came on and they were to the left a bit, the approach lights were to the planes 1 or 2 oclock. The first officer said “lights to the right” to which the captain responds with “lights in sight”, as the plane was put into a slight right turn, The flight engineer called minimum decision height, the captain decided to continue with the landing. The captain thought that the runway was a bit too low but he thought that he could still make the approach and decided to continue. He pushed the throttles a bit forward to compensate for the planes low altitude. But despite that the first officer and the flight engineer were telling the captain that the plane was too low. As flight 933 was landing an air canada plane was waiting to takeoff and the captain of the air canada plane said that the DC 10 was too low to recover. The plane was quote desperately low. But what the captain was doing was not working. Within a few second the flight engineer was rapidly said 50 40 30 20 10 as fast as he could and then the plane struck the the approach lights. In the tower the approach lighting system audio alarm was activated. On the radio frequency someone said “Iberia 933 we have an accident” From the controller saw a trail of fire behind the plane, along with an explosion he then reached for the crash alarm as soon as soon. The DC 10 lifted off again after losing its right gear, the captain could no longer see the runway and he pushed the control column forward and then settled back down on the runway hard . And another, the left side of the plane burst into flames. The plane then came to a stop. Thankfully no one onboard the aircraft was hurt and everyone walked away from the crash, but flight 933 had left a trail of destruction in its wake, the approach lights to the runway were destroyed. And the runway had taken a beating. But nothing that couldnt be fixed. Right after the crash the localizer and the glideslope were tested, this seemed like a good idea because the plane was low on approach after all but the thing is the tests found out that the beacons were within tolerances. So that wasn't a cause of the crash of flight 933. But that being said the charts that they had stated clearly that the glideslope wasnt usable below 200 feet that's something to keep in mind I guess. So to figure out what happened they did the next best thing, they got a simulator and programmed in the conditions of that day and got five pilots to fly a few approaches to the airport. In each of the simulated landings they noticed that when the autopilot was disconnected at 200 feet the rate of descent of the giant jet would spike all the way to 1150 feet per minute, now thats very fast. They also realized that the pilots only had a short 6 second window to apply corrective measures, if they didnt then the plane would come crashing down well before the threshold of the runway. But what was very interesting is that in none of the simulator sessions did they get the plane to be not lined up with the runway. You remember how on the accident approach the plane wasnt really lined up with the runway?

Korean Air Flight 801 - 228 dead: Despite protests from the flight engineer that the captain was not detecting the correct signal for landing, he pressed on and guided flight 801 from Seoul, South Korea directly into the Guam mountainside leaving only 26 survivors. Air France Flight 447 - 228 dead: 1 June 2009 flight 447 disappeared on its way to Paris from Rio de Janeiro. Although the wreckage was found after 5 days the black box was recovered 2 years later. The cause was found to be an inappropriate pilot response to faulty air speed indicators due to bad weather conditions and ice. Swissair Flight 111 - 229 dead:On 2 September 1998 Swissair flight 111 from New York to Geneva crashed into the ocean just off of Nova Scotia due to a rapidly spreading cockpit fire. 9/11 flights:Although not a single air disaster, the four flights involved in 9/11 were responsible for the greatest single day loss of life in commercial aviation history. Two of the flights, American Airlines Flight 11 and United Airlines Flight 175 each were responsible for thousands of casualties after being driven into the World Trade Centers and are considered the largest and second largest losses of life due to individual plane crashes in all of aviation history with 1700 and 1000 people killed respectively. Tenerife Airport Disaster - 583 dead: In terms of passengers and crew killed this collision between two Boeing 747s on the Spanish Island of Tenerife is considered the deadliest plane crash in history. Ermenonville air disaster - 346 dead:After a Moroccan baggage handler was unable to read the proper latching instructions in either English or Turkish on Turkish airlines flight 981, the latch blew off mid flight as the plane crashed into Ermonville forest near Paris, France. Furthermore, the latches had a known design flaw. Air India Flight 182 - 329 dead:On the 23 of June, 1985 a bomb exploded onboard Air India Flight 182 as it was flying between London and Montreal. The plane crashed into the Atlantic Ocean just off the coast of Ireland. A Canadian Sikh militant group was responsible. Saudia Flight 163 - 301 dead:This flight from Rihadh to Jeddah caught fire shortly after takeoff. It turned around and made an emergency landing but everyone on board died of smoke inhalation before fire crews could get inside. The fire was started due to butane stoves used by Muslim pilgrims because of their strict dietary restrictions. Saudi Arabia has since banned the stoves but some people still smuggle them onboard to make tea. Iran Air Flight 655 - 290 dead: During the Iraq-Iran war on July 3, 1988 the USS Vincennes shot down Iran Air Flight 665 going between Tehran and Dubai due to confused communications. American Airlines Flight 191 - 273 dead:Just moments after takeoff the left engine of Flight 191 from Chicago to Los Angeles came off. The plane rolled, turned upside down and crashed into a field. A firefighter on the scene was quoted as saying that not a single complete body was found. Amateur photos of the crash sparked a media firestorm condemning the DC-10’s poor safety record.

American Airlines Flight 587 was a regularly scheduled international passenger flight from New York's John F. Kennedy International Airport to Las Américas International Airport in Santo Domingo, the capital of the Dominican Republic. On November 12, 2001, the Airbus A300B4-605R flying the route crashed shortly after takeoff into the Belle Harbor neighbourhood of Queens, a borough of New York City. All 260 people aboard the plane (251 passengers and 9 crew members) were killed, along with 5 people on the ground. It is the second-deadliest aviation incident involving an Airbus A300 and the third-deadliest aviation incident to occur on U.S. soil. The location of the accident and the fact that it took place two months and one day after the September 11 attacks on the World Trade Center in Manhattan initially spawned fears of another terrorist attack. Terrorism was officially ruled out as the cause by the National Transportation Safety Board (NTSB), which instead attributed the disaster to the first officer's overuse of rudder controls in response to wake turbulence from a Japan Airlines (JAL) Boeing 747-400 that took off minutes before it. According to the NTSB, the aggressive use of the rudder controls by the first officer caused the vertical stabiliser to snap off the plane, along with the plane's two engines separating from intense forces before impact. Because the crash occurred just two months and one day after the September 11 attacks in New York, several major buildings including the Empire State Building and the United Nations Headquarters were evacuated. In the months after the crash, rumors circulated that the plane had been destroyed in a terrorist plot, with a shoe bomb similar to the one found on Richard Reid. In May 2002, a Kuwaiti national named Mohammed Jabarah agreed to cooperate with investigators as part of a plea bargain. Among the details Jabarah gave authorities was a claim made to Jabarah by Khalid Sheikh Mohammed's lieutenant, who told Jabarah that Reid and Abderraouf Jdey had both been enlisted by the al-Qaeda chief to carry out identical shoe-bombing plots as part of a second wave of attacks against the United States. According to this lieutenant, Jdey's bomb had successfully blown up Flight 587, while Reid's attempt had been foiled. In May 2002, a Canadian government memo was written that repeated the claims suggesting that Jdey had a role in the crash, while conceding that the reliability of the source of that information — Khalid Sheikh Mohammed's lieutenant — was unknown. According to information contained in the memo, Jdey — a naturalized Canadian citizen — was to use his own Canadian passport to board the flight. While American Airlines' passenger manifest did indicate citizens boarding with passports from the United States, the Dominican Republic, Taiwan, France,[d] Haiti, and Israel, no passengers boarded using a Canadian passport. According to NTSB spokesman Ted Lopatkiewicz, the weight of the memo's veracity was put into question, as no evidence of a terrorist traveling on board was found. The evidence suggested that the aircraft was brought down after a piece of the empennage, "the vertical fin, came off", while it did not indicate "any kind of event in the cabin."

The Crash: The flight left Surabaya 15 minutes late at 5:35am. After departure, the flight was in contact with Jakarta Centre, which provides ATC for most of Indonesia. At 6:12am, the aircraft was flying at 32,000 feet and began to approach violent storms. The pilots contacted ATC to fly a different route and fly at a higher altitude to avoid the storms. ATC deferred Flight 8501 but said they could climb to 34,000 feet but no response was heard. At 6:17am, the aircraft began to pitch upwards from 32,000 feet to 37,000 feet at a rate of 6,000 feet per minute which twice the maximum rate a commercial aircraft should be climbing. At just over 36,000 feet, the jet then began to descend. The plane descended 8,000 feet in half a minute and began to turn left. The airplane then ditched into the sea killing everyone on board. The crash was caused by a rudder failure. The pilots also misunderstood each other during the emergency, causing them to pitch the aircraft up even further, worsening the stall.

Mais au bout de quelques secondes de vol, une première alarme se déclenche : les indicateurs d’altitude et de vitesse sont bloqués. D’autres incidents et alarmes s’enchaînent et bientôt, le commandant Eric Schreiber et son copilote ne maîtrisent plus l’appareil. L’avion essaie de retrouver sa route vers l’aéroport mais le contrôleur aérien reçoit des données erronées en provenance de l’ordinateur de bord et ne peut pas secourir l’équipage. Soudain, l’avertisseur de proximité du sol se déclenche. L’appareil court droit à la catastrophe. Après 30 minutes de vol, l’avion s’écrase dans l’Océan Pacifique. Tous les passagers et membres de l’équipage périssent dans la tragédie. L’enquête commence aussitôt. Elle révèle rapidement une erreur humaine à l’origine du crash du Boeing : les prises d’air statique, permettant de mesurer l’altitude, étaient obstruées par un ruban adhésif oublié par l’équipe de maintenance. Cette tragédie amène également à s’interroger sur le comportement de l’équipage et sur la part de responsabilité des pilotes par rapport à l’informatisation du matériel. En effet, les pilotes doivent parfois oublier les ordinateurs et s’en remettre à leur propre expérience. Ce documentaire permet de revivre les conditions réelles de ce crash aérien. American Airlines Flight 191 was a regularly scheduled passenger flight operated by American Airlines from O'Hare International Airport in Chicago, Illinois, to Los Angeles International Airport in Los Angeles, California. On May 25, 1979, the McDonnell Douglas DC-10-10 operating this flight was taking off from runway 32R when it crashed into the ground. All 258 passengers and 13 crew on board were killed, along with two people on the ground. With 273 fatalities, it is the deadliest aviation accident to have occurred in the United States. The National Transportation Safety Board (NTSB) found that as the aircraft was beginning its takeoff rotation, engine number one (the left engine) separated from the left wing, flipping over the top of the wing and landing on the runway. As the engine separated from the aircraft, it severed hydraulic fluid lines that lock the wing's leading-edge slats in place and damaged a 3-foot (1 m) section of the left wing's leading edge. Aerodynamic forces acting on the wing resulted in an uncommanded retraction of the outboard slats. As the aircraft began to climb, the damaged left wing – with no engine – produced far less lift (stalled) than the right wing, with its slats still deployed and its engine providing full takeoff thrust. The disrupted and unbalanced aerodynamics of the aircraft caused it to roll abruptly to the left until it was partially inverted, reaching a bank angle of 112 degrees, before crashing in an open field by a trailer park near the end of the runway. The engine separation was attributed to damage to the pylon structure holding the engine to the wing, caused by improper maintenance procedures used at American Airlines. The aircraft involved was a McDonnell Douglas DC-10-10 registered N110AA. It had been delivered on February 25, 1972, and at the time of the crash, it had logged just under 20,000 hours of flying time over seven years. The jet was powered by three General Electric CF6-6D engines. A review of the aircraft's flight logs and maintenance records showed that no mechanical discrepancies were noted for May 11, 1979. On the day of the accident, in violation of standard procedure, the records were not removed from the aircraft, and were destroyed in the accident.

The plane b737 max started the flight from Addis Ababa airport (HOOB airport code) with 157 people onboard. For this flight the flightnumber was ET302. At the day and time when the plane took off, the weather seems turbulent and a few clouds around Addis Ababa airport. The plane started the flight early morning and took off from Bole airport runway 07L at 08:38 a.m. local time. 6 minutes After takeoff the pilots reporting tecnical difficulties and asked for clearance to return. At 08:44 a.m. local time flight ET302 went down near Bishoftu, southeast of Addis Ababa airport. At the time that the plane went down you can hear people onboard screaming, this sound is added while editing, so this is not implemented from the simulator. All 157 people aboard an Ethiopian Airlines flight that crashed soon after taking off from Addis Ababa have been killed. The pilot was a senior Ethiopian Airlines pilot who had flown more than 8,000 hours. He had an "excellent flying record. I wish a lot of strength for the families of the victims. Two fighter aircraft of the Indian Air Force crashed on Saturday, defence ministry sources said. Preliminary reports suggested that the aircraft crashed near Morena in Madhya Pradesh. Defence Minister Rajnath Singh was briefed by Chief of Air Staff Air Chief Marshal V R Chaudhari on the crash of two IAF aircraft. The aircraft- a Sukhoi-30 was carrying two pilots and the Mirage 2000 was carrying one pilot during the time of the crash.
